Probably because of the lack of AIDS/HIV vaccination and the number of rape in the third world countries.

It's the lack of education and awareness in third world countries that makes HIV/AIDS more rampant there. Also, it's the lack of health care and availability of doctors. Without the right tools and education, it's difficult for any society to do anything. If they were educated in the importance of using condoms, or how HIV/AIDS are spread, or how being HIV/AIDS means that your children will be born with it too. All these are things that aren't always taught in third world countries. Also, HIV/AIDS medication is extremely overpriced and simply not a viable option for most people in third world countries.

The biggest reason is because women do not have rights in third world countries. They cannot refuse sex or they could be beaten or killed. Also men in 3rd world countries prize virgins and very young women, so there would be more blood involved. Rape is very common. Also, in some countries "dry sex" is preferred, which leads to bleeding. When women are able to have a voice in their future, the rates will start dropping.

i think the main cause is due to the lack of contraception, health awareness etc in these countries. in some remote areas they simply donot have access to simple things such as contraception, health infor and certainly are not receiving advice/awareness enough about this disease. poor sanitory conditions donot help either. there is also a problem with drug users sharing needles, prob in dirty unclean conditions, and people just live in this unfortunate cycle as they donot know how or dont have the opportunity, to get themselves out of these conditions.
